Pre-holiday Survey

  • You are required to complete this short impact survey before your holiday. The answers you provide help us show how holidays make a difference and allows us to get future support and funding for families.

  • The form takes around 5-10 minutes to complete, and if you need any support, please contact the person who referred you for the holiday.

  • There are no right and wrong answers so please answer how you feel is best. Your responses will be used in line with our privacy policy and may be shared anonymously. See 'More information' for further details.

  • All families that complete the pre & post holiday questions will have the option to be entered into a draw to be in with a chance of winning a £100 voucher. The voucher can be redeemed at a number of well-known supermarkets and food retailers.
